Blue Hill Health is an independent medical practice that attempts to create a medical home.

 

The Patient Centered Medical Home (PCMH)  is a healthcare setting that creates partnerships between individual patients and their personal physician, and, when appropriate, the patient’s family. A PCMH can be any primary care medical practice, hospital, or clinic that follows a certain set of principles. In February 2007, four primary care societies—American Academy of Family Physicians, American Academy of Pediatrics, American College of Physicians and the American Osteopathic Association—developed these principles, the Joint Principles for the PCMH, which are summarized in these points: 

  • Ongoing relationship with a personal physican;

  • Physician-directed medical practice;

  • Whole-person orientation;

  • Care is coordinated and/or integrated;

  • Quality and safety;

  • Enhanced access to care; and

  • Payment appropriately recognizes the added value
